The Nurse Navigator is a registered nurse who serves as the central point of contact for patients and families moving through the care continuum. The Navigator identifies and removes clinical, financial, logistical, and psychosocial barriers to timely care; coordinates appointments, testing, and referrals across providers and settings; and provides education and support from initial contact through treatment and follow-up.

Working within a multidisciplinary team, the Navigator promotes continuity of care, improves patient understanding of the treatment plan, supports adherence, and contributes to reduced avoidable emergency department visits and readmissions. The role blends direct patient interaction, care coordination, and program-level quality work.

Patient Navigation and Care Coordination

<ul>

	Serve as the consistent, identified point of contact for assigned patients and their caregivers across the episode of care.

	<li>
	Conduct initial and ongoing assessments to identify clinical, financial, transportation, language, health literacy, cultural, and psychosocial barriers to care.

	Track pending results and outstanding orders; ensure findings are communicated to the ordering provider and, as appropriate, to the patient.

Patient Education and Support

<ul>
	<li>Provide evidence-based education on diagnosis, treatment options, medications, self-management, symptom monitoring, and warning signs requiring escalation.</li>
	<li>Deliver education at an appropriate health literacy level and in the patient's preferred language, using interpreter services when needed.</li>
	<li>Support shared decision-making by helping patients formulate questions, understand options, and communicate their goals and preferences to the care team.</li>
	<li>Provide emotional support and reinforce coping strategies; recognize distress and refer to social work, behavioral health, chaplaincy, or community resources.</li>
	<li>Connect patients to financial counseling, insurance and prior authorization assistance, prescription assistance programs, transportation, nutrition support, and other community resources.</li>
	<li>Educate patients on advance care planning and advance directives where appropriate.</li>

Documentation, Quality, and Program Development

<ul>
	<li>Document all navigation activities, assessments, interventions, communications, and outcomes accurately and timely in the electronic health record.</li>
	<li>Collect and report navigation metrics — for example, time to treatment, barriers identified and resolved, no-show rates, patient volume, and patient satisfaction.</li>
	<li>Participate in quality improvement initiatives, process improvement projects, and accreditation or certification survey readiness.</li>
	<li>Contribute to the development of navigation protocols, pathways, patient education materials, and standard work.</li>
	<li>Participate in community outreach, screening events, and education programs as assigned.</li>

The Occupational Health &amp; Safety Nurse will be responsible for providing nursing and first-aid support, assisting with employee health and wellness initiatives, supporting workplace health and safety programs, and maintaining accurate health-related records.

As part of the role, the successful candidate will also provide front-office and administrative support, including assisting employees, visitors, applicants, and guests and handling basic reception and documentation requirements.

Key Responsibilities

1. Employee Health &amp; Nursing Support 

<ul>
	<li>Provide first aid and basic nursing care to employees.</li>
	<li>Conduct basic health assessments and respond appropriately to employee health concerns.</li>
	<li>Perform IV Therapy procedures, including insertion, monitoring, and discontinuation of intravenous lines, in accordance with company protocols and nursing standards.</li>
	<li>Monitor employee health conditions and provide appropriate initial assistance.</li>
	<li>Coordinate medical referrals and hospital assistance when necessary.</li>
	<li>Maintain confidential and accurate employee medical records.</li>
	<li>Monitor and follow up on laboratory results, fit-to-work certificates, and medical clearances.</li>
	<li>Maintain and monitor medical supplies and equipment.</li>
	<li>Assist in health-related concerns and employee wellness activities.</li>
</ul>

2. Occupational Health, Safety &amp; Wellness

<ul>
	<li>Assist in implementing workplace health and wellness programs.</li>
	<li>Support occupational health and safety initiatives.</li>
	<li>Assist in identifying and reporting workplace health and safety concerns.</li>
	<li>Conduct or assist with workplace health and safety inspections.</li>
	<li>Coordinate with HR and the Safety Officer on employee health and safety matters.</li>
	<li>Support health awareness campaigns, seminars, and wellness activities.</li>
	<li>Monitor workplace sanitation and basic health and safety practices.</li>
	<li>Assist in maintaining compliance with applicable occupational health and safety requirements. </li>

3. Front Office &amp; Administrative Support 

<ul>
	<li>Greet and assist employees, visitors, applicants, and guests professionally.</li>
	<li>Direct visitors and guests to the appropriate person or department.</li>
	<li>Provide front-office support as part of daily workplace operations.</li>
	<li>Answer phone calls and assist with basic inquiries.</li>
	<li>Receive deliveries and coordinate them with the appropriate personnel.</li>
	<li>Maintain an organized and professional reception/front-office area.</li>
	<li>Assist with filing, data entry, document processing, photocopying, and scanning.</li>
	<li>Maintain organized health-related and administrative records.</li>
	<li>Provide related administrative support to HR and other departments when necessary.</li>

Occupational Health and Safety Nurse

	<li>Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care</li>
	<li>Prepare patients for and assist with examinations or treatments</li>
	<li>Administer medications as prescribed by physicians</li>
	<li>Monitor, record, changes in patients' conditions and report symptoms</li>
	<li>Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition</li>
